STTA and Administration of Medication Policy

As per the board's strict policy on administration of medication during the school day, please find attached appendices that require a parent/guardian's attention if medication for your child is required during the school day.

In the first attachment:
Appendix Four is to be completed by the Physician.
Appendix Five is to be completed by the Parent/Guardian.
Appendix Six is to be completed by the school each time medication is administered.

When a child brings medication to school (prescription and non-prescription), it MUST be kept in the office in a secure location. If the medication is prescribed, it must be kept in the original container with the appropriate Pharmacist labels. The label will describe the name, DIN, dosage, dosage limits and administration instructions. If the medication is over the counter, it must also be kept in the original container along with a signed note from the parent/guardian regarding administration instructions.

When medication is dispensed at school, each incident will be recorded in the log as per Appendix Six.

The completion of the second attachment is required for those students who require an epipen for Anaphylactic reactions.
The completion of the third attachment is required for those students who require an inhaler for Asthma.
